Starting a Home Health Care Agency and Getting Your Home Health Care Agency License

B. Obtain copy of DOH License

C. (if applicable) Start Medicaid Application

E. Send Medicaid Application out to the state DOH

Step 6. CHAP Accreditation for Medicare or Private Duty

A. Apply to chap

B. OASIS Test Transmittal

C. CLIA Waiver application

D. Submit Survey Readiness
